Which status effect causes damage over time and is strongly associated with Caelid in Elden Ring?

The story behind the answer

Scarlet Rot is the damage-over-time status effect strongly associated with Caelid in Elden Ring.

Scarlet Rot rapidly drains a target’s health after its buildup bar is filled. Caelid is marked by the influence of the rot, with crimson landscapes, diseased wildlife, and the aftermath of the battle between Malenia and Starscourge Radahn. The region is therefore the game’s most recognizable setting for this affliction.

Scarlet Rot is not the same as Poison. Both deal damage over time, but they are separate status effects with different buildup and resistance interactions. Blood Loss instead causes a large burst of damage after sufficient buildup, while Frostbite temporarily affects damage and stamina-related behavior.

Players can resist or cure Scarlet Rot with suitable equipment, consumables, spells, and the preserving boluses item. The status also appears in areas beyond Caelid, including the Lake of Rot. Its lore connection to Malenia makes it one of the game’s most important environmental and narrative themes.
