The first John Madden Football release in 1988 was for the Apple II.
Electronic Arts’ original game was developed for Apple II computers after a lengthy effort to create a detailed football simulation. Designer Robin Antonick worked with EA founder Trip Hawkins and John Madden, whose insistence on realism helped define the project. Madden famously wanted 11 players on each side rather than a simplified six- or seven-player format.
The Apple II version included individual plays, player fatigue, injuries, penalties, and customizable game conditions. It did not yet have official NFL teams or player names, because EA had not secured NFL or NFLPA licenses. Later versions were ported to MS-DOS and Commodore 64 in 1989.
The Apple II original is sometimes overlooked because the franchise became much more famous on consoles. The 1990 Sega Genesis game was a separate release using the same John Madden Football name, and it became the breakthrough console version. Amiga and other platforms came later, so they cannot be the answer to the 1988 first-release question.
