Who was the lunar module pilot on Apollo 16?

Charles Duke was the lunar module pilot on Apollo 16.

Apollo 16 launched in April 1972 with John Young as commander, Charles Duke as lunar module pilot, and Ken Mattingly as command module pilot. Young and Duke descended to the lunar surface in Orion, while Mattingly operated the command module Casper in orbit.

Duke and Young explored the Descartes Highlands for more than 20 hours across three surface excursions. Duke collected samples, operated instruments, and drove the lunar roving vehicle with Young. He was 36 years old during the mission and remains the youngest person to walk on the Moon.

Duke is sometimes confused with Apollo 14 lunar module pilot Edgar Mitchell or Apollo 15 lunar module pilot James Irwin. Apollo 16 was the fifth mission to land astronauts on the Moon.
