Wolverine accidentally releases Omega Red in season 2, episode 5 of X-Men '97. Wolverine and Morph recruit members of Logan's former Team X, including Sabretooth, Lady Deathstrike, Maverick, and Garrison Kane, to raid a ruined Weapon X facility.
Wolverine's stated mission is to assassinate scientist Abraham Cornelius, but his private goal is to restore his adamantium skeleton. While undergoing the procedure, he becomes infected by Brood organisms. The infection drives him into a violent state, during which he kills Maverick and inadvertently releases Omega Red.
Omega Red is a particularly fitting threat for Wolverine because the two are linked to the Weapon X era and Soviet super-soldier experiments. After the Brood infection is purged, Wolverine regains his adamantium claws and defeats Omega Red. The common mix-up is blaming the release on Cornelius or the facility's collapse; the episode identifies Wolverine's infected actions as the direct cause.
