House M.D. is modeled partly on the fictional detective Sherlock Holmes.

The connection appears in several parts of the series. Gregory House solves medical mysteries through observation, deduction, and elimination, much as Holmes solves criminal cases. House and Holmes also share a willingness to pursue a logical explanation even when their methods frustrate colleagues or violate social expectations.

The names contain deliberate echoes: House resembles Holmes, and James Wilson parallels Dr. John Watson, Holmes's friend and companion. House lives at 221B Baker Street for part of the series, directly referencing Holmes's famous London address. Both characters also play musical instruments and have difficult relationships with drugs.

The influence does not mean House is a direct adaptation of a particular Holmes story. The series translates detective fiction into a hospital setting, with diseases replacing crimes and diagnostic tests replacing investigative evidence. The medical cases therefore function as mysteries while remaining grounded in a contemporary teaching hospital.
